You would be surprised at what designers don't consider. Most of the site designs our professional services group gets from "professional designers" for implementation come in the form of graphical comps or a basic HTML sample page. They often do not consider multiple screen sizes, non-javascript browsers, and other usability factors. If you are using a design firm that has a good compliment of skilled HTML staff (and not just GUI editor jockies) and graphical designers then you generally get a good result.
